Pediatric Toe Walking: What Parents Should Know
What is Toe Walking?

Many young children walk on their tiptoes as they learn to walk. In most cases, occasional toe walking is a normal part of development and resolves on its own. However, if a child continues to walk on their toes after age 2 or 3, does it frequently, or is unable to walk with their heels down, it may be time for an evaluation.
Toe walking occurs when a child walks primarily on the balls of their feet without making normal heel contact with the ground. Some children toe walk only occasionally, while others do it almost all the time.
Persistent toe walking can be associated with:
- Habitual (idiopathic) toe walking
- Tight calf muscles or Achilles tendons
- Sensory processing differences
- Muscle weakness
- Balance and coordination challenges
- Neurological conditions
- Developmental delays
- Autism spectrum disorder (in some children)
- Cerebral palsy
- Muscular dystrophy
While toe walking does not always indicate a medical condition, persistent toe walking should be evaluated to identify the cause.
Pediatric Toe Walking Signs & Symptoms
Physical signs
Parents may notice their child:
- Walks primarily on their tiptoes
- Has difficulty placing their heels flat on the floor
- Walks on their toes more than 50% of the time
- Has tight calf muscles
- Frequently trips or falls
- Has poor balance
- Complains of foot or leg pain
- Walks with stiff legs
- Has decreased ankle flexibility
- Tires easily during physical activity
- Has difficulty running, jumping, or climbing stairs
- Avoids walking barefoot
Additional Signs That May Need Evaluation

- Continues toe walking after age 2–3
- Cannot voluntarily walk flat-footed
- Walks on only one foot
- Has increasing muscle tightness
- Begins toe walking suddenly after previously walking normally
