What is Plagiocephaly or "Flat Head Syndrome"?

Plagiocephaly, often called “flat head syndrome,” is a condition where a baby develops a flattened area on one side or the back of the head. This commonly happens when infants spend extended periods lying in one position or have tight neck muscles (torticollis) that make it difficult to turn their head evenly. Early intervention through pediatric physical and occupational therapy can make a significant difference in improving head shape, neck mobility, strength, and overall development.

Pediatric Plagiocephaly Signs & Symptoms

Plagiocephaly, also called “flat head syndrome,” can range from mild to more noticeable. Many babies with plagiocephaly are otherwise healthy, but parents may begin noticing changes in their baby’s head shape or movement preferences during the first few months of life.

Common Symptoms of Plagiocephaly:

A baby’s head may appear:

  • Asymmetrical (uneven shaped head)
  • Slanted or uneven
  • Wider on one side
  • Flattened back of the head
  • Parallelogram-shaped when viewed from above

 

Babies may consistently:

  • Turn their head to one side
  • Sleep facing the same direction
  • Resist turning their head the opposite way

This can sometimes occur alongside torticollis (tight neck muscles).

 

One ear may appear:

  • Slightly pushed forward (uneven ear positioning)
  • Higher or more forward than the other ear

 

In more noticeable cases, parents may observe:

  • Uneven cheeks (facial asymmetry) 
  • One eye appearing slightly smaller
  • Uneven forehead prominence

 

Babies with plagiocephaly often have:

  • Tight Neck Muscles (Torticollis)

  • Difficulty turning their head both directions
  • Neck stiffness
  • Head tilting to one side

 

Some infants may also experience mild delays with:

  • Delays in Gross Motor Development 
  • Tummy time tolerance
  • Rolling
  • Sitting
  • Crawling
  • Head control

Not every baby with plagiocephaly will have developmental delays, but early therapy support can help encourage symmetrical development.

 

Parents may notice:

  • Their baby always sleeps with their head turned one way
  • Difficulty with tummy time
